The reason I told you that you should only wait a month at the smaller sizes is because the jump to the next size is much smaller than it is at bigger sizes. I got the timeline from Bodyartforms Forums, which to my knowledge is a very reputable website dealing with stretched ears.

It may not be the most accurate but it does give people new to stretching a pretty good idea of how long to wait.

Not necessarily. You could encounter problems later. Especially a build up of scar tissue that will make future stretches a lot more difficult and you will build wrinkles in your ears known as the "cat butt" effect. I've been through it before. It should be 5-6 months from a fresh piercing to your first stretch. You should downsize immediately back to 18G and and do sea salt soaks. If your not familiar with that, take a cup or shot glass and add 1/8 tsp. of non-iodized sea salt to 1 cup of warm water and soak your piercing for 5-10 minutes ...

Yes, you use the taper to stretch then hold the plug to the taper, like an extension of the taper, then slide it through with the taper.
Yes, a titanium plug sounds fine. ONLY use steel, titanium, or glass jewelry in your piercing after the fresh stretch while it heals.

You should use the taper only to stretch, not to wear as jewelry. You should put in the taper and then put in your jewelry. But being that you've had your ears pierced for so long, I would suggest that you start at 16G. Usually ears are pierced with 20G or 18G so I think it would be safe to start ...

If your ears started bleeding, I would suggest that you downsize to 4G. Do sea salt soaks until your ears are healed and wait for a while longer then you can stretch back up to 2G. Then you should wait around 4 more months to 0G. This may seem like a lot, but to be safe and to keep your lobes happy ...
